How to Apply
Application Procedures
Send a scanned copy of the following to mcsdtec-admissions@mc.edu.ph
- PSA Birth Certificate
- Form 138 – Report Card (If available, otherwise, it may be sent later)
- Recent passport size photo
Below are modes of payment:
BDO Deposit Referencing (over the counter) or Bills Payment (thru online)
- Account Number: BDO SA# 003570-0072-05
- Account Name: Miriam College Foundation, Inc.
- Reference Number: Applicant No. / Student ID No.
Receive Acknowledgement and the details of the Official Receipt (OR) from the Treasury Office.
Present original admission requirements to the Office of Academic Support and Student Services for enrollment confirmation and sectioning.
Admission Requirements:
- PSA Birth Certificate (original)
- Report Card (Form 138) – original
- Transcript of Records (TOR/Form 137)
- Character Reference from last school attended / sponsoring organization or Barangay/Police Clearance
- Recommendation Form from the Guidance Counselor
- ITR or Certification with salary from employer (if applicant is employed) / ITR or Certification with salary of parents (if applicant is unemployed)
- Photocopy of PSA Marriage Certificate (if married)
- Passport size pictures (white background with name)
- Chest x-ray (film and result) – to be submitted during enrollment
- Junior High School Certificate (for SHS students)
- QVA Certificate (for SHS students from private school)
