Request for School Documents

Students and alumni may request school documents such as diplomas, Transcript of Records (TOR), certificates, and authenticated documents from the Registrar’s Office. To know the types and cost of documents, please view the Request Form below:

1. Download and accomplish the Registrar's Request Form.

2. Fill out the form until Purpose of the Request ONLY and email the accomplished form to college_registrar@mc.edu.ph
  • If you are requesting for a Certified True Copy (CTC) of documents (TOR, Diploma), please submit a CLEAR PHOTOCOPY or SCANNED COPY IN PDF format. 
  • For Ched Authentication and Verification (CAV) requests, kindly submit the Original copy of Transcript of Records (TOR) and a photocopy of Diploma (for graduates only).
  • For International Credential Assessment Service of Canada (ICAS), International Credential Evaluation Service (ICES), International Qualifications Assessment Service (IQAS), NASBA International Evaluation Services (NASBA), and World Education Services (WES) requests, kindly attach a copy of your transcript together with an accomplished request form from the verifying agency. 
  • For other documents not included in the form above, please email college_registrar@mc.edu.ph to make a request.
3. The Registrar’s Office will review your request and supporting documents. You will receive instructions for payment and claim stub.

THE REGISTRAR’S OFFICE WILL REVIEW YOUR REQUEST AND SUPPORTING DOCUMENTS. YOU WILL RECEIVE INSTRUCTIONS FOR PAYMENT AND CLAIM STUB.

4. For payment of requests, please see bank details below:

Plan Holder’s Name: Name of Student

Email a clear copy of the deposit slip or confirmation page with your COMPLETE NAME to mctreasury@mc.edu.ph for validation. Cross copy mcaccounting@mc.edu.ph
and college_registrar@mc.edu.ph for records.

5. Acknowledment of Payment

You will receive an acknowledgment of payment from the TREASURY OFFICE.

If you do not receive an acknowledgement after 3 working days, you may call  09285120279 or send a follow up letter to mctreasury@mc.edu.ph.

IMPORTANT REMINDERS FOR CLAIMING OF DOCUMENTS

  • Present the claim stub together with the Official Receipt/ acknowledgement from the Treasury Office and valid I.D. when claiming the requested document/s.
  • Representatives must present a signed AUTHORIZATION LETTER, I.D. CARDS (of owner & representative) in addition to the above requirements.
  • Document/s not claimed after six (6) months counting from the stated date of release will no longer be valid and payments made will be forfeited.

Request for Recommendation Letter

Students and alumni may request recommendation letters from the HEU Guidance & Counseling Office. Recommendation letters are usually required for on-the-job training and application for a job or new school.

Request for School Documents

To request for recommendation letters, follow the instructions below:

1. Download and accomplish the Recommendation Letter Request Form.

2. Email the accomplished form

Email the accomplished form to the HEU Guidance & Counseling Office emails mcheuguidance@mc.edu.ph.

3. Pay the processing fee at:

Request for GOOD MORAL CERTIFICATE

The Good Moral Certificate is provided by the HEU Office for Student Affairs. To request for this certificate, please email Ms. Susa Marasigan of the HEU Office of Student Affairs (OSA) at pmarasigan@mc.edu.ph
